Pre-Admission
If you cases are not emergency cases, here are simple reminders that patients can do before getting admitted to our institution:
- Prepare an identification card or a government ID wherein personal information could be validated
- Prepare your health information regarding your current condition like co-morbidities and current medications that are being taken.
- Prepare and bring your insurance documents or medical coverage ID cards
- Bring your healthcare cards or insurance coverage forms, as well as any special form(s) or pre-authorization documents such as Letter of Guarantee (LOG) or Letter of Authority (LOA).
Important Documents
We encourage our patients to have with you the following documents (as applicable):
- Any personal Identification (ID) card with photo:
- Accredited HMO-issued member’s ID
- Senior Citizen’s ID
- PhilHealth ID
- Persons with Disability ID
- Healthcare insurance card or document
- Digitized SSS ID
- GSIS ID
- PRC ID
- Driver’s License
- Passport
- Any recent or prior X-Ray films, laboratory results or medical records (especially if you had a recent hospital confinement at another medical facility or hospital)
- A copy of your doctor’s advance directive regarding your admission or medical care (if available)
Patient’s Rights
The hospital respects the rights of the patients as follows:
- The right to a considerate and compassionate care
- The right to know information concerning diagnosis, treatment, prognosis, procedures as well as the risk and benefits involved in the treatment
- The right to know the identity of physician, nurses and others involved in the care of the patient
- The right to know implications of treatment of choice
- The right to make decisions about the plan of care
- The right to have an advance directive concerning treatment
- The right to protect their privacy
- The right to treat all communications and records with confidentiality
- The right to have access and review records pertaining to the patient’s medical care.
Patients’ Responsibilities
- To provide an accurate information about his/her health condition
- To cooperate with health care team
- To be accountable for his/her health in the progress of the treatment
- To respond positively to the doctor’s orders or requests.
- To be aware of prohibited business relationships
- To be aware and comply with hospital policies, rules and regulations
- To be aware of hospital charges, pay hospital bills and avail of the payment methods.
House Rules:
- Always maintain cleanliness and orderliness in the room, ward and around the hospital premises
- Smoking and alcoholic beverages are not allowed within the hospital premises
- Destruction and vandalism of hospital property will be dealt with accordingly
- Notify the nursing staff for any damage facility
- Bringing home of fixtures from inside the room is prohibited
- Unauthorized person is not allowed inside the room
Policy regarding companions:
- Children ages 10 years old and below are not allowed inside the patient’s room
- Only one companion is allowed
- Antigen nasal swab/RT-PCR nasal swab is required for companions of patients
